IBH Care Coordinator (Integrated Behavioral Health)

Location: Oakland, CA

Schedule: Monday – Friday, 8:00 a.m. – 5:00 p.m.

Salary Range: $66,000 – $80,000, based on experience

About the Role

This role bridges behavioral health and social services, helping clients access the support they need across care systems. Ideal for ASWs/ACSWs ready to apply their clinical knowledge in a fast-paced, community-focused setting. Includes clinical supervision and mentorship.

Responsibilities Include

1. Provide hybrid care coordination services (in-person and remote)
2. Conduct assessments and co-develop care plans
3. Navigate referrals across housing, medical, and behavioral health sectors
4. Support follow-up and long-term engagement
5. Work alongside clinical teams and external agencies
6. Record progress and interventions in the EHR system
7. Support reporting, compliance, and outreach efforts

Who You Are

1. Registered ASW/ACSW in California
2. Degree in social work, psychology, or related field
3. 1+ year of experience in social services or behavioral care
4. Familiar with Alameda County health and housing networks
5. Attentive to documentation, timelines, and compliance standards

Bonus if You Have:

1. Primary care or integrated care background
2. Bilingual capabilities
3. SmartCare or similar EHR familiarity

Working Conditions

1. Split time between clinic and community
2. Light travel throughout Alameda County
3. Occasional evening flexibility
