???? Schedule: Exempt, min 40 hours per week | 2:30pm - 11pm | Monday - Friday with an on call rotation.

???? What You’ll Do in This Role:

As the Clinical Supervisor in our Assisted Living community, you’ll provide compassionate leadership and oversight to ensure residents receive high-quality, person-centered care. You’ll coordinate and evaluate individualized care plans, support and guide staff, ensure regulatory compliance, and collaborate with families and healthcare providers to promote resident wellness in a respectful and supportive environment.

Primary Responsibilities:

Resident Care Oversight:

  • Develop, implement, and evaluate individualized care plans for each resident.
  • Monitor and assess residents' physical, emotional, and social needs, adjusting care plans as necessary.
  • Ensure timely and accurate documentation provided and changes in residents’ conditions.
  • Support the team by rounding the floor, assisting, and delivering direct care as needed.

Resident and Family Communication:

  • Act as a liaison between residents, families, and the interdisciplinary team.
  • Address and resolve concerns or issues raised by residents or their families.
  • Provide regular updates to families regarding residents’ care plans.

Regulatory Compliance:

  • Maintain up-to-date knowledge of Home for the Aged (HFA) regulations and best practices.
  • Participate in inspections and audits, implementing corrective actions as needed.

Quality Improvement

  • Monitor and assess the quality of care and services provided to residents.
  • Identify areas for improvement and implement strategies to enhance care delivery.
  • Conduct and review incident reports, ensuring that corrective measures are taken.
  • Audit and ensure accurate documentation in resident charts and care plans.

Emergency Response

  • Oversee and manage emergency situations to ensure resident safety and well-being.
  • Coordinate emergency services when necessary.

What You’ll Need:

Qualifications & Requirements:

  • Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure in the State of Michigan.
  • Minimum of two (2) years of experience in memory and dementia care preferred.
  • Minimum of three (3) years of experience working with the elderly population preferred.
  • Proficiency with electronic medical records; experience with PointClickCare is a plus.
  • On-call support required, including participation in a rotating weekend coverage schedule.
  • Must be able to lift and move objects weighing up to 35 pounds without assistance.
