Job Description:
You will be providing services by monitoring, assessing, and providing directed treatment common to complex injuries and illnesses through comprehensive assessments, including diagnosis, treatment, and outcome management for adult and pediatric patients.
- Each day, you will work to provide care and monitoring for patients
- Engage in assessments and provide treatment under the supervision of an Advanced Practice Provider (APP) for patients facing barriers to accessing the traditional healthcare system
- Throughout the treatment process, maintain patient medical records and documentation consistent with state regulations, our company standards, and policy through the appropriate EHR system(s)
- You will be practicing under direction with synchronous telemedicine support
Responsibilities:
- Act as a conduit for the medical treatment of our patient population programs
- Navigate conversations related to the management of health, well-being, medication management, and goals of care
- Provide supervised, directed treatment of common medical diagnoses
- Participate in continuing education as required by the state where you reside and the training team
- Adhere to clinical and safety standards, protocols, and performance metrics
- Provide care with cultural sensitivity and compassion
- Lead your practice with a patient-centered approach
- Comfort with Point of care testing and diagnostics including but not limited to retinal imaging and bone density measurements
- Comfort with technology and utilization of EHR systems per program
- Other related duties as deemed necessary
Qualifications:
- Minimum of two years of experience as a Licensed Practical Nurse
- Active and current American Heart Association BLS Required / ACLS certification Preferred
- Emergency Department, Urgent Care, Internal Medicine, Hospital – Critical Care, Family Practice, or other relevant areas experience preferred
- Current unrestricted LPN license
- Clinical work experience with pediatrics (preferred)
- If hired, you must complete third-party Phlebotomy training
Salary:
$60,000 - $80,000 per year
