Position: Family Medicine Physician
Location: Mechanicville, New York
Employment Type: Full-Time
Schedule: Monday – Friday, 9:00 AM – 5:00 PM
Salary: $250,000 – $300,000 per year + benefits

You will have a direct hand in improving health outcomes and supporting families within our community while also collaborating with other healthcare providers and specialists to coordinate patient care.

Key Responsibilities:

  • Provide primary care services for patients of all ages, from infants to elderly adults, addressing acute and chronic conditions across the lifespan.
  • Conduct annual physical exams, screenings, vaccinations, and health risk assessments to promote wellness and prevent disease.
  • Diagnose, treat, and monitor long-term conditions such as diabetes, hypertension, asthma, and obesity, working closely with patients to develop personalized care plans.
  • Diagnose and treat a wide variety of acute health conditions, including infections, injuries, and short-term illnesses, ensuring timely and effective care.
  • Educate patients on healthy lifestyle choices, such as nutrition, exercise, stress management, and smoking cessation, to promote long-term health and prevent disease.
  • Collaborate with specialists, nurses, and other healthcare providers to ensure coordinated care, including referrals, follow-up care, and management of complex medical conditions.
  • Serve as a trusted advocate for patients, helping them navigate the healthcare system, understand treatment options, and make informed decisions about their care.
  • Provide continuity of care for individuals and families, fostering long-term relationships and delivering care that addresses both individual and family health needs.
  • Provide mental health screenings, address common issues such as anxiety and depression, and refer patients to appropriate behavioral health specialists when necessary.
  • Ensure accurate, timely documentation of patient visits, treatment plans, and follow-up care in accordance with healthcare regulations, including confidentiality and HIPAA compliance.
  • Engage in quality improvement initiatives, tracking patient outcomes, and recommending improvements in care delivery to enhance patient satisfaction and health outcomes.
  • Participate in community outreach efforts, health education programs, and wellness clinics to promote health and engage with local populations.

Qualifications:

  • Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree.
  • Current and unrestricted New York medical license.
  • Board certification in Family Medicine.
  • Completion of the U.S. Medical Licensing Examination (required)
  • 5+ years of experience as a Family Practitioner
  • Exceptional counseling, listening, and communication skills
  • Strong organizational, time management, and decision-making abilities
  • Ability to perform under pressure in a fast-paced environment
  • A compassionate approach that puts patients at ease
  • Excellent leadership and management skills
