IDD NURSE - CADILLAC

This is a professional position assigned within a variety of settings, including the interdisciplinary team and case management. The Nurse primarily works in residential group homes (both Agency operated and contractual), family homes and day program facilities. It also involves work with a variety of populations across the entire lifespan, but primarily with developmentally disabled persons served. Although the expectations for the position may vary from program to program, the overall primary function for this position is to ensure that nursing/health programming is appropriate and suitable and consistent with generally accepted standards of persons served care. This position reports to the Operations Manager, as assigned. This position has no direct supervision responsibilities.
ESSENTIAL FUNCTIONS
- Design, administer, evaluate and update nursing/health care programs as appropriate.
- Conduct nursing appraisals and health screenings as appropriate.
- Facilitate coordination of persons served care between and among NLCMHA psychiatrists, primary care, providers, consulting specialists, and other providers as needed.
- Review persons served care for quality and appropriateness of health interventions. Report suspected deficiencies in persons served care to supervisors and work collaboratively to find solutions to improve care quality within the scope of the Nurse's responsibilities.
- Participate in interdisciplinary staffing meetings and other pertinent persons served care meetings and/or staff meetings.
- Provide ongoing case consultation to other members of the interdisciplinary team, including Agency staff and contractual providers, relative to nursing and health care services.
- Provide information and resources to persons served, caregivers, guardians and staff relative to specific diagnoses, and health conditions.
- Administer medications and perform other medical procedures such as injections, as persons served needs dictate, in accordance with applicable policies/procedures and nursing standards of practice.
- Conduct AIMS/DISCUS testing.
- May deliver medications to persons served as requested and observe setup of persons served medications.
- Participate in Quality improvement case review procedures, including person served/ guardian satisfaction.
- Maintain knowledge of mental health symptomatology, intervention and treatment strategies, particularly as they apply to the developmentally disabled individuals.
- Provide occasional crisis intervention services related to treatment of persons served health issues. (In the event the nurse consultant may need to be incorporated into the formal crisis services system for mental health care, the Agency will provide adequate training.)
- Provide urgent health/nursing services to assigned persons served as needed and other registered persons served within the IDD Team.
- Coordinate with other agency staff procurement of adaptive equipment and medical/dental services.
QUALIFICATIONS
- Nursing degree
- Two (2) years of experience in providing services to consumers who have mental health needs and developmental disabilities is preferred.
- Training and expertise in geriatric, day programming and/or consultation may be required.
- Registered Nurse Licensure with the State of Michigan
- Additional certification may be required
- Credentialed by NLCMHA to provide services in this professional discipline
PAY: $64,782 - $74,282
