Position Overview

We are seeking a dedicated and compassionate Speech-Language Pathologist (SLP) to join our team at Forward Function Healthcare Center in Salem, Oregon. Work Environment

Your work will be primarily performed in our skilled nursing facility, where you will collaborate closely with a interdisciplinary team to deliver integrated care. Key Responsibilities

• Conduct comprehensive assessments to identify speech, language, cognitive-communication, and swallowing difficulties in residents.
• Develop and implement individualized treatment plans utilizing evidence-based practices tailored to each resident's unique needs.
• Collaborate with the multidisciplinary team, including nurses, occupational therapists, and dieticians, to coordinate care strategies that improve residents' functional abilities.
• Provide training and guidance to residents, their families, and caregivers on effective communication techniques and safe swallowing practices.
• Document patient progress meticulously and update treatment plans regularly based on evolving clinical data and outcomes.
• Adhere to all professional and regulatory standards to ensure the highest quality of care is consistently maintained.

Patient Population / Client Focus

You will work primarily with elderly residents facing various challenges, including neurodegenerative diseases, stroke recovery, and age-related communication barriers. Your expertise will be crucial in aiding these individuals to communicate effectively and safely, ultimately enhancing their experiences within the nursing facility.

Qualifications

• Master’s or Doctorate degree in Speech-Language Pathology from an accredited institution.
• Current and valid Speech-Language Pathology licensure in the state of Oregon.
• A minimum of two years of clinical experience, preferably in a skilled nursing or rehabilitation setting.
• Excellent interpersonal and communication skills complemented by a patient-centered approach to care.

Preferred Experience

While not mandatory, candidates with experience in geriatric rehabilitation or experience utilizing advanced therapeutic tools and modalities will be preferred. Familiarity with electronic health records (EHR) systems is also advantageous for seamless documentation and communication across the care team.

Compensation and Benefits
