Job Description
Position Overview
As a Home Healthcare LPN, you’ll deliver skilled nursing care and clinical support to patients in their residences, promoting independence, comfort, and quality of life. You’ll work closely with Registered Nurses (RNs), therapists, physicians, and family members to implement individualized care plans and monitor patient progress.
Key Responsibilities
Clinical Nursing Care
Administer prescribed medications, injections, and IV therapy per physician orders.
Perform wound care, catheter care, ostomy care, and other nursing procedures.
Monitor vital signs and assess patients’ physical and mental status.
Observe, record, and report changes in patient condition to supervising RN or physician.
Care Planning & Documentation
Contribute to the development and revision of individualized care plans.
Maintain accurate, timely documentation in the electronic health record (EHR) or paper chart.
Complete visit notes, medication logs, and incident reports in compliance with agency policies and state regulations.
Patient & Family Education
Instruct patients and caregivers on medication administration, symptom management, and use of medical equipment.
Provide guidance on nutrition, hygiene, and activity levels consistent with the care plan.
Reinforce safety measures and emergency protocols.
Coordination & Collaboration
Communicate effectively with RNs, physical/occupational therapists, social workers, and physician offices.
Coordinate referrals, lab draws, and specialist visits as needed.
Participate in interdisciplinary team meetings and case conferences.
Quality & Compliance
Adhere to all infection control standards (e.g., hand hygiene, PPE use).
Follow state nurse practice act, HIPAA, and agency policies.
Report any adverse events, near misses, or potential safety issues promptly.
Qualifications & Skills
Licensure: Current LPN license in [State] (active and unencumbered).
Education: Graduate of an accredited practical nursing program.
Experience: Minimum 1 year of LPN experience; home health experience preferred.
Certifications:
CPR/First Aid (BLS) – American Heart Association or equivalent.
Additional certifications (e.g., IV certification, wound care) a plus.

Physical Requirements
Ability to lift, transfer, and reposition patients safely (up to 50 lbs).
Stamina to stand, bend, and move throughout the home for extended periods.
Comfortable driving between client locations; valid driver’s license and reliable vehicle required.
