What You’ll Do:

• Provide therapy sessions for individuals managing chronic health challenges.
• Develop treatment plans and monitor progress.
• Serve as the primary point of contact for patients and their care teams.
• Personalize care strategies to meet patient-specific needs.
• Offer non-medical counseling and conduct risk assessments.
• Educate patients and their families on available resources.
• Ensure accurate record-keeping and compliance with standards.

Salary: $110,000 to $130,000 annually, plus benefits

Qualifications:

• Master’s in Social Work from an accredited program.
• Licensed as an LCSW in California.
• A minimum of 2 years of clinical experience focused on psychotherapy.
• Knowledge of solution-focused therapeutic methods.
• Strong communication and problem-solving abilities.
