Job Description
Christie Clinic's department of OB/GYN is seeking a full-time Licensed Clinical Social Worker(Sign-On Bonus Available) from Monday-Friday, 8:00am-5:00pm at the University clinic.
Duties include collaborating with the Internal Medicine physician team to implement and manage individualized treatment plans for patients facing a variety of conditions, including but not limited to, mood disorders, anxiety disorders, PTSD, phobias, and addictive behaviors.
A Master's in Social Work and a Licensed Clinical Social Worker license required. 3-5 years of post-licensure social work experience preferred.
JOB DUTIES: (This list may not include all of the duties assigned.)
- Provide one-on-one therapeutic support to patients’ based on provider referral.
- Provide patient centered care in group setting based on provider referral.
- Assist patients and families in understanding diagnosis, treatment options and resources available to them.
- Serves as a liaison in planning care between patients, families and multi-disciplinary health care providers.
- Provide educational and informational resources to patient and family to optimize resources and care.
- Complete assessments with patients to identify expressed, implied or anticipated needs.
- Complete mandated reports of suspected abuse and/or neglect of minors, disabled adults, and seniors. Cooperate and collaborate with the investigating bodies for safe plans.
- Provide empathic listening and emotional support to patients in person.
- Provide assessments of patients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and/or life situation.
- Educates patient and family members in a manner that overcomes barriers; matches their learning capabilities and meets fundamental needs.
- Ability to make psychosocial assessments and develop and implement care plans
- Visit notes and orders are completed and transmitted in a timely manner according to policy. Corrections to care plans are entered and transmitted according to time line.
- Facilitates and supports patient decisions and communication of self-determined life care decisions.
- Documents patient/family assessments, financial assessment and MSW interventions within patients' electronic medical record.
- Provides care according to plan of care/orders. Develops social work plan of care in collaboration with IDT.
- Conduct face to face interviews and assessments with patients as needed.
- Maintain information by patient in written and electronic form.
- Perform related work as required.
TYPICAL PHYSICAL DEMANDS:
Demands include sitting, standing, walking, bending, stooping, stretching, and lifting up to 75 pounds. Hearing within, or correctable to, normal range, vision correctable to 20/20 and manual dexterity for the operation of office equipment is required.
TYPICAL WORKING CONDITIONS:
Work is performed in a clinical setting with possible exposure to communicable diseases, toxic substances, hazardous waste, bodily fluids and other waste common to a health care environment. Ionizing radiation and medicinal preparations are also common. Requirements may include working irregular hours.
PAY AND BENEFITS:
The estimated pay range for this position is exclusive of fringe benefits and potential bonuses. Final offers are based on various factors, including skill set, experience, qualifications, and other job-related criteria.
