Job Summary
We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our team. The ideal candidate will provide high-quality patient care to an individaul that resides alone. You will be responsible for monitoring patients' health, Bladder flush, administering medications, and collaborating with healthcare professionals to ensure optimal patient outcomes. Your expertise in nursing practices and patient assessment will be essential in delivering exceptional care to the individual.
Responsibilities
- Provide direct patient care, including administering medications and treatments as prescribed by physicians.
- Monitor and document patients' vital signs, medical history, and overall health status.
- Assist with physical examinations and diagnostic evaluations.
- Collaborate with healthcare teams to develop and implement individualized care plans.
- Educate patients and the support team on health management and disease prevention.
- Maintain accurate medical records and documentation in compliance with HIPAA regulations.
- Perform procedures such as daily Bladder Flush and catheterization
- Support patients with activities of daily living (ADLs) while promoting independence.
Skills
The successful candidate should possess a range of skills that enhance patient care quality, including but not limited to:
- Proficiency in EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks.
- Experience in critical care settings (ICU/PICU), emergency medicine, or urgent care environments.
- Knowledge of ICD-9/ICD-10 coding for accurate medical documentation.
- Familiarity with various aspects of patient care including geriatrics, pediatrics, hospice care, and memory care.
- Strong assessment skills for evaluating patient needs and implementing appropriate interventions.
- Ability to perform basic life support (BLS) and administer injections safely.
- Understanding of infection control practices and aseptic techniques.
- Excellent communication skills for effective interaction with patients, families, and healthcare teams.
