What You'll Be Doing

• Provide skilled nursing visits in patients' homes

• Assess patient conditions and monitor progress

• Administer treatments and interventions as ordered

• Educate patients and caregivers on disease management and care plans

• Collaborate with physicians and interdisciplinary team members

• Complete timely and accurate clinical documentation

• Participate in an on-call rotation

Qualifications

• Active Texas LVN/LPN license

• Minimum 6 months of recent home health experience required

• At least 1 year of nursing experience

• Current CPR certification

• Valid driver's license and reliable transportation

• Strong assessment, communication, and documentation skills

Schedule

Full-Time

Monday–Sunday shifts available

8:00 AM – 5:00 PM

On-call rotation required

Pay Details: $71,000.00 to $87,000.00 per year
