Licensed Practical Nurse

Licensed Practical Nurse (LPN) – Long Term Care (4 Open Roles)
Location: Brooklyn, NY
Pay Rate: $34–$35/hour

We are hiring 4 experienced Licensed Practical Nurses (LPNs) with strong nursing home / long-term care experience to provide quality resident care in a skilled nursing environment. The LPN will work under the direction of a Registered Nurse or physician while supporting assessments, treatments, documentation, and individualized care plans.

Responsibilities:

• Provide direct nursing care to residents in a long-term care setting.
• Perform assessments, monitor resident conditions, and report changes in status.
• Administer treatments and support individualized care plans.
• Maintain accurate and timely clinical documentation.
• Collaborate with RNs, physicians, care managers, and interdisciplinary teams.
• Assist with discharge planning and coordination of continued care needs.
• Maintain resident safety, privacy, and compliance with facility standards.

Requirements:

• Current NY State LPN License required.
• Minimum 1 year nursing home / LTC experience required.
• Strong clinical skills and professionalism.
• Excellent communication and documentation skills.
• EPIC experience preferred.

Shifts Available:

1. Days: 7:00 AM – 3:15 PM (Every Other Weekend)
2. Evenings: 3:00 PM – 11:15 PM (Every Other Weekend)
3. Nights: 11:00 PM – 7:15 AM (Every Other Weekend)
4. Weekend-only roles available
