Summit Home Care and Hospice is searching for an experienced and motivated LPN to join our Home Health & Hospice teams. At Summit, we believe healthcare should be personal, compassionate, and delivered where it matters most - at home.
Licensed Practical Nurse (LPN):
- Hours: Monday - Friday; Standard Business Hours & Occasional On-Call Rotation
- Work Setting: On the Road for Patient Visits & Training at the Local Office or Remotely
- Territory: ~30-Mile Radius (Mileage Reimbursed)
- Pay Rate: $28.00 - $31.00 per Hour
- Location: Harrison, OH & Surrounding Areas
Responsibilities: - Provide direct patient care under the supervision of a registered nurse.
- Follow the plan of care, provide treatments, and work collaboratively with the members of the team to help meet positive patient care outcomes.
- Implement the plan of care initiated by the registered nurse.
- Provide accurate and timely documentation consistent with the plan of care.
- Assess and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
- Participate in coordination of home health and hospice services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinic Manager.
- Use and/or prepare equipment and supplies effectively and efficiently adhere to aseptic techniques.
- Participate in personal and professional growth and development.
- Perform other duties assigned by the registered nurse, including caring for hospice patients as census allows.
- Participate in weekend and holiday on-call rotation requirements.
Qualifications: - Must be a graduate from an accredited practical nurse or vocational nursing program.
- Must hold a valid/active license as a Practical Nurse (LPN) or Vocational Nurse (LVN) in Ohio.
- 3 years of nursing experience, preferred.
- Prior experience in community/home health and/or medical/surgical, preferred.
- Comply with accepted professional standards and practices.
- Demonstrates good verbal and written communication, and organizational skills.
- Must be able to lift, position or transfer patients and lift supplies and equipment.
