Position Overview

Trinity Health Behavioral Center is seeking dedicated Licensed Professional Counselors (LPC) for full-time and part-time positions to support our growing client base through telehealth services. This role allows counselors to work remotely, providing therapy and guidance to individuals facing mental health challenges in a flexible, patient-centered environment. The competitive salary range for this position is USD 87,000 – 105,000 annually, reflective of experience and qualifications.

Key Responsibilities

• Conduct comprehensive patient intakes, focusing on diagnostic assessments and functional evaluations to formulate individualized treatment goals.
• Provide brief, targeted interventions for clients requiring mental health services, ensuring alignment with the established Agency and Federally Qualified Health Center (FQHC) guidelines.
• Collaborate closely with primary care providers to manage and address psychosocial issues, acting as an integral member of a multidisciplinary team.
• Initiate follow-up procedures to monitor patient progress, adapting treatment plans based on their evolving needs.
• Craft and implement relapse prevention strategies while assisting clients in maintaining stable functioning.
• Identify patients at risk and develop tailored intervention plans to prevent the deterioration of their mental health conditions.
• Coordinate behavioral health care services across various platforms, linking patients with both on-site and off-site treatment providers with their consent.
• Maintain up-to-date, thorough clinical documentation regarding patient progress, treatment plans, and therapeutic interventions.
• Participate in organizational initiatives, providing insights and recommendations for program development and outcome evaluation.
• Promote patient self-management skills by equipping them with educational resources and coping strategies related to their treatment.
• Ensure adherence to the Code of Ethical Conduct for Clinical Professionals and carry out other relevant duties as assigned.

Qualifications

• A Master’s Degree in Clinical Counseling is required.
• Current LPC (Licensed Professional Counselor) License is mandatory.

• Outstanding capability to build empathetic, professional relationships with clients, families, and healthcare teams.
• Strong foundational knowledge of community resources and their effective use in a behavioral health context.

• Proficient in maintaining clinical records and preparing necessary reports.

Preferred Experience

• Bi-lingual proficiency in English and Spanish is preferred, facilitating enhanced communication with a wider patient base.

• Familiarity with telehealth platforms and technology is advantageous.
