Work Environment

This position is fully remote, utilizing advanced telehealth technology to connect with clients from various geographical areas. You will engage with patients in their own environments, facilitating comfort and openness during sessions. Key Responsibilities

• Conduct comprehensive assessments to determine the clinical needs of patients, establishing baselines for treatment planning.
• Maintain clear communication with team supervisors, direct care staff, and families regarding patient progress, coordinating updates for a collaborative care approach.
• Establish and present a detailed schedule of appointments and home visits to supervisors to ensure consistent monitoring of patient advancements.
• Coordinate home visits that include observations and data analysis to fully understand clients’ living conditions and challenges.
• Oversee the implementation of individualized treatment plans, ensuring all interventions are effective and compliant with established protocols.
• Actively participate in interdisciplinary team meetings, sharing insights and strategies to enhance patient outcomes.
• Document monthly and quarterly progress notes, ensuring thorough records are maintained to track patient development.
• Organize and develop training sessions for staff to ensure competent execution of treatment plans and compliance with documentation standards.
• Provide follow-up assessments and continued support through home visits, ensuring long-term success and adaptation of treatment as necessary.
• Serve as a liaison with the broader mental health community, promoting LifeStance’s goals and supporting community mental health initiatives.

Patient Population / Client Focus

In this role, you will primarily work with a diverse client population, including adults and youths facing a range of mental health challenges. Through personalized telehealth sessions, your expertise will support individuals with issues related to anxiety, depression, trauma, family dynamics, and substance misuse. You will foster a safe and supportive environment tailored to each client's unique circumstances, promoting healing and growth.

Qualifications

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.
• Must possess an active, unrestricted license as a Professional Clinical Counselor (LPCC) in California.
• A minimum of two years of counseling experience in a clinical context is essential.
• Specializations in family therapy, couples therapy, trauma, or substance misuse are highly recommended.
• Strong familiarity with legal and ethical standards in the mental health profession is crucial.
• Exceptional interpersonal and communication skills to effectively engage diverse clients are necessary.
• A proven background in collaborating with a healthcare team to achieve comprehensive patient care.
• Proficient in using Electronic Medical Records (EMR) software and related technologies for administrative tasks.

Preferred Experience
