Key Responsibilities

Your day-to-day responsibilities will include:

• Conducting initial assessments and ongoing evaluations to tailor treatment plans to individual patient needs.
• Facilitating effective communication between supervisors, caregivers, and other stakeholders to ensure a comprehensive understanding of each patient's development and progress.
• Creating and sharing treatment plans with supervisors, detailing appointments, home visits, and observations to monitor client progress and satisfaction.
• Arranging and executing home visits for in-depth evaluations and observations, as necessary, to capture the full scope of the client's circumstances.
• Participating in interdisciplinary team meetings and training sessions to enhance collaboration and share best practices across the clinic.
• Providing timely and thorough documentation of client progress through monthly and quarterly notes, ensuring adherence to legal and ethical standards.
• Identifying opportunities for training and professional development for both staff and clients, enhancing the effectiveness of treatment programs.
• Monitoring client well-being through follow-up interviews and observant in-home assessments after initial treatment to adjust care as needed.
• Acting as a bridge between the clinic and the community, connecting clients with resources and support networks.

Patient Population / Client Focus

In this role, you will work with a varied client base, including both adults and youth, addressing challenges ranging from emotional distress to substance abuse and trauma recovery. Your work will help empower clients to improve their mental health and quality of life by offering personalized treatment approaches grounded in evidence-based practices.

Qualifications

• A Master's or Doctoral degree in Counseling, Psychology, or a related field.
• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Arizona.
• A minimum of two years of counseling experience in a clinical setting.

Preferred Experience

• Specialization in family therapy, couples therapy, substance misuse, or trauma is highly preferred.
• Demonstrated understanding of legal and ethical standards in mental health practice.
• Exceptional interpersonal and communication skills, with the ability to establish rapport with clients from diverse backgrounds.
• Proficiency in using Electronic Medical Records (EMR) software and technologies relevant to telehealth counseling.

Compensation and Benefits
