Key Responsibilities

• Conduct comprehensive patient intakes, focusing on diagnostic assessments and functional evaluations, while developing tailored treatment recommendations for primary care providers.
• Deliver brief, focused therapeutic interventions for patients needing immediate mental health services, ensuring that all interventions align with organizational guidelines and best practices.
• Regularly monitor patient progress and collaborate with primary care providers to adjust treatment plans as necessary, contributing to improved patient outcomes.
• Assist primary care providers in recognizing and managing psychosocial issues, enhancing their capacity to treat patients holistically.
• Develop and implement relapse prevention plans for at-risk patients to maintain their stability and well-being.
• Document patient progress meticulously in treatment charts, maintaining detailed records of diagnoses and treatment plans.
• Share insights and recommendations with clinic management and fellow healthcare providers, contributing to program evaluation and enhancing service delivery.
• Promote self-management skills among patients, empowering them to participate actively in their treatment and recovery processes.

Patient Population / Client Focus

You will engage with a diverse patient population, including individuals grappling with various mental health disorders, emotional challenges, and behavioral issues. Your role will focus on assessing individual needs and providing compassionate, evidence-based therapeutic interventions to help patients achieve better mental health outcomes. Understanding the personal and social implications of mental health issues is vital, as you will also support patients' families and connect them with relevant community resources.

Qualifications

• A Master’s Degree in Clinical Counseling or a related field is required.
• A valid Licensed Professional Counselor (LPC) license is mandatory.

• Familiarity with DSM-IV diagnoses and the ability to apply them in a clinical context.
