Zepf Center has been serving the Lucas County community for over 50 years. We are the leading provider of behavioral health and substance use disorder services in Northwest Ohio. Services include adult and child psychiatric, substance abuse, case management, residential, Crisis Care, and therapy programs, as well as career development and wellness services. Zepf Center also offers primary care medical services to our patients to contribute to their continuum of care. Zepf Center is a trauma-informed agency and environment for both patients and staff.


We are currently seeking a Full Time LPN to serve as the licensed practical nurse in the crisis stabilization unit. The individual in this role will serve as the licensed practical nurse in the crisis stabilization unit. The focus of this program is to provide inpatient, crisis services that support individuals experiencing a mental health crisis. The CSU is utilized for individuals undergoing an acute crisis that presents a substantial risk of imminent harm to self or others. The goal is to stabilize the individual through nursing and behavioral supports on a time-limited basis. The role of the licensed practical nurse is to provide nursing care to individuals in the 24-hour CSU as part of the interdisciplinary treatment team. Shifts are 12 hours.


Essential Duties of this position include but are not limited to:


Hours: 3, 12-hour shifts weekly; 6:00PM-6:00AM with rotating weekends and assigned holidays

  • Responsible for assessing individuals.
  • Carries out doctor’s orders including medication administration and laboratory draws, and evaluates all treatment provided.
  • Conducts therapeutic groups and provides documentation that meets the standard level of care on each shift worked.
  • Reviews files of any admissions from previous shift with attention to any medical issues, medical documentation, and ensure appropriate follow-up.
  • Reviews all shift reports from last time worked and ensure follow-up of any noted medical issues.
  • Completes vital signs on all patients as policy and prudent judgment indicates.
  • Ensures medications are available and provided to upcoming shifts for distribution to patient.
  • Documents medical information appropriately in patient records and shift report according to established policies
  • Provides assistance to incoming shift, ensuring all pertinent medical information is given.
  • Ensures appropriate referral services for patients/applicants to a hospital for necessary medical services, as prudently indicated.
  • Provides education to patients regarding medical issues, providing self-care; appropriate taking of medications, disease of addiction, etc.
  • Intervenes appropriately in crisis situations, using appropriate resources, including 911.
  • Performs other duties as assigned.

  • Knowledge and support of agency mission vision goals and philosophy as well as the policies and procedures.
  • Demonstrate knowledge and ability to implement care within the framework of standard nursing practice and agency policy and procedure.
  • Knowledge and ability to administer and dispense medication in accordance with physicians’ orders.
  • Knowledge of community resources, hospitals, community mental health services, and other appropriate referrals for use when indicated.
  • Ability to obtain and maintain licensure and provide documentation for personnel records.

Requirements:
  • Must be a LPN in good standing and have certification allowing the administration of medication.
  • Must have a valid driver's license.
  • Must be able to function positively and effectively with patient, residents, the Medical Director, and other staff members.
  • CPR and First Aid trained and crisis intervention (CPI) certified.
  • Knowledge and adherence of CARF standards.
  • The ability to work with minimal supervision.
  • Ability to write clear, concise, accurate notes, and correspondence with the laws of confidentiality.
  • Must be able to maintain the confidentiality of the work.
  • Must be able to function in a changing environment effectively.
  • Must have the medical knowledge and experience to accomplish the responsibilities.
  • Must have the necessary skills and abilities required to comfortably and effectively working with patients, being responsive to their needs.
