Key Responsibilities

• Provide exceptional primary care services, encompassing office visits for routine and acute medical issues.
• Conduct annual wellness visits and health risk assessments, employing a holistic approach to healthcare that emphasizes thorough and accurate documentation.
• Educate patients about their health conditions including anatomy, risk factors, and warning signs, promoting active engagement in their care plans.
• Coordinate care with other healthcare providers, specialists, and testing facilities, ensuring seamless transitions and comprehensive patient management.
• Lead population health initiatives, working closely with our Care Team to ensure patients receive timely screenings, such as adhering to colorectal cancer screening guidelines.
• Engage in regular patient care rounds and conferences, effectively communicating strategies and collaborating with team members to optimize patient outcomes.

Patient Population / Client Focus

Qualifications

• A Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree from an accredited medical school.

Preferred Experience

• Familiarity with electronic health record (EHR) systems to facilitate thorough documentation and care continuity.

Compensation and Benefits
