Well Vida Healthcare is in search of a dedicated and compassionate Family Medicine Practitioner to join our team in Rochester, New York. This full-time role requires a multifaceted approach as the physician will provide comprehensive primary care services to patients of all ages within an outpatient setting. Work Environment

This position is primarily onsite in our practice with a focus on outpatient care. Our state-of-the-art facility is designed to promote a positive patient experience, equipped with advanced diagnostic equipment and support technologies. As part of a supportive medical team, you will work closely with nurses, medical assistants, and other healthcare professionals to ensure seamless delivery of care. Your work schedule will be Monday through Friday from 9:00 AM to 5:00 PM, allowing for work-life balance.

Key Responsibilities

• Conduct comprehensive assessments of patients’ medical histories, evaluating their age-specific needs and prescribing appropriate interventions.
• Document clinical observations, diagnoses, treatment plans, and evaluate patient responses to treatments meticulously to ensure continuity of care.
• Develop personalized care plans that address physical, psychological, cultural, and social factors affecting patient health.
• Initiate and direct relevant laboratory and diagnostic procedures, ensuring timely follow-up interventions based on results.
• Perform essential medical procedures such as venipuncture and on-site laboratory diagnostics to support immediate patient care.
• Educate patients in regard to their health needs, identifying gaps in knowledge and utilizing appropriate resources to address them.
• Work collaboratively with the medical team to coordinate patient care, ensuring that all interventions reflect best practices and enhance care continuity.

Patient Population / Client Focus

As a Family Medicine Practitioner at Well Vida Healthcare, you will have the opportunity to impact a diverse patient demographic—from pediatrics to geriatrics. You will address a variety of chronic and acute health conditions, emphasizing preventive care and health promotion. Qualifications

• Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree, or a foreign equivalent.
• Completion of a U.S. accredited residency program in Family Medicine is essential.
• Board Eligible or Board Certified in Family Medicine (BE/BC).
• A valid and unrestricted New York State medical license.
• Active enrollment in Medicare/Medicaid or eligibility for enrollment.
• Current Basic Life Support (BLS) or Advanced Cardiovascular Life Support (ACLS) certification.
• Minimum of 2 years of experience preferred, though new residents may be considered.

Preferred Experience

• Demonstrated experience in chronic disease management, particularly with conditions such as diabetes and hypertension.
• Expertise in preventive medicine and promoting health within diverse populations.
• A proven ability to thrive in a multidisciplinary team environment, promoting open communication and collaborative care.
