Work Environment

This position is primarily based onsite at our well-equipped hospital facility in Utica, New York. We foster a collaborative and supportive environment where healthcare professionals work closely together to ensure the best possible patient outcomes. The role operates on a predictable schedule of Monday to Friday from 9 AM to 5 PM, allowing for work-life balance without the need for regular weekend coverage.

Key Responsibilities

• Provide comprehensive medical care to adult patients, particularly within the geriatric population, incorporating evidence-based practices.
• Conduct thorough assessments, including detailed medical histories, physical examinations, and necessary diagnostic testing.
• Develop, implement, and monitor individualized treatment plans tailored to each patient's needs, goals, and preferences.
• Prescribe medications and therapies, ensuring understanding and adherence to treatment protocols among patients and their families.
• Collaborate with multidisciplinary teams, including nurses, social workers, and specialists, to coordinate holistic patient care and ensure continuity of services.
• Engage in patient education to promote disease prevention, health promotion, and effective self-management strategies.

Patient Population / Client Focus

The physician will primarily care for older adults who often contend with complex health issues requiring comprehensive and coordinated care. Our practice emphasizes a patient-centered approach that respects the values and preferences of our geriatric patients, encouraging their active involvement in their healthcare decisions.

Qualifications

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
• Active Board Certification in Family Practice or Internal Medicine.
• Valid and unrestricted medical license to practice in New York State, along with a current DEA registration.

Preferred Experience

• A minimum of 2 years of relevant experience in internal medicine or primary care with a focus on geriatrics.
• Proven experience in health coaching, patient education, and utilizing electronic health record (EHR) systems effectively.
• Demonstrated ability to build strong relationships with patients and families, enhancing their adherence to treatment plans.
• Excellent communication skills, both verbal and written, to convey complex medical information in an accessible manner.

Compensation and Benefits
