SUMMARY:

Working directly with people who have a diagnosed mental illness, the Mental Health Counselor will assess the client’s personal, medical, emotional, social, and environmental situations to plan for treatment course. The MHC (mental health counselor) works within a team oriented, strengths-based approach, ensuring collaboration with the team to directly provide services to adults recovering from mental illness.

The MHC possesses a strong foundation in trauma informed care, motivational interviewing, and other evidenced-based approaches. The counselor is responsible for treatment plans and assessments along with providing group and individual counseling/therapy.

Duties:

  1. Directly provide evidenced based counseling to treat adults and youth diagnosed with mental illness in group and individual sessions. Sessions focus on mental health - physical, emotional, creative, environmental, occupational, financial, social, and spiritual wellness.
  2. Directly provide services to meet needs in areas of community integration, skill building, social interactions, coping with symptoms and connecting to resources. Complete case management tasks with clients, including benefit eligibility.
  3. Provide an approach to clinical services that fosters hope for recovery.
  4. Carry a caseload, meet performance / productivity goals (minimum of 20 documented service hours a week).
  5. Complete and update IM+ CANS and IM + CANS reviews on time and with accuracy.
  6. Communicate with external stakeholders such as family and medical coordinators to provide continuity of care.
  7. Accept and process referrals. Maintain positive relations with referral sources.
  8. Keep service/progress notes using an electronic record keeping system and document services weekly (daily is preferred).
  9. Provide crisis intervention, including facilitating hospitalization, as necessary
  10. Provide emergency assistance/program coverage during scheduled hours, and at other sites as required. Respond to medical and behavioral / psychiatric crisis in accordance with agency policy and procedure. This includes all related documentation.
  11. Work as a team with other staff to provide required services for clients, including internal referrals.
  12. Promote consumer driven, mental health services and enhance the abilities of clients to make choices.
  13. Maintain case files according to agency / regulatory standards.
  14. Develop and implement curricula and activities based on the changing needs of the clients. Track attendance and evaluate client’s progress.
  15. Provide alternate coverage for other teams as needed.
  16. Communicate effectively and professionally with all stakeholders. Share information concerning clients with other employees’ as relevant within confidentiality rules.
  17. Assist clients with transportation needs.
  18. Plan and participate in special events, community activities, and family events.
  19. Adhere to all agency safety policies and procedures, including universal precautions, abuse and neglect prevention and reporting, evacuation procedures and guidelines for intern or subcontract work.
  20. Maintain a flexible working schedule while performing duties. This may include the need to travel off-site or to rearrange the scheduling of breaks and lunch to meet the program's immediate needs.
  21. Maintain a professional demeanor in both manner and attire as a representative of the agency in conducting all activities, services, and instructions.
  22. Complete all competency-based training as required.
  23. Actively participate in required supervision.
  24. Conduct and monitor safety drills as needed per Anixter policies.
  25. Other duties as required.

REQUIREMENTS

  • MHP required (bachelor’s level). QMHP (Qualified Mental Health Professional – master's level) per DHS MH (Mental Health) Rule 132 and 140 preferred.
  • Ability to be IM+ CANS (MH assessment) certified within 6 months of employment
  • 6 months experience working with people who have a diagnosis of mental illness.
  • Computer proficiency required - mastery and use of electronic healthcare records required.
  • Valid driver’s license and ability to meet Agency insurance requirements for Wilson program.
