Work Environment

Key Responsibilities

• Conduct thorough psychosocial assessments through interviews, observations, and review of medical records to identify emotional, social, and environmental factors influencing patients' recovery.
• Provide compassionate counseling and emotional support to patients and their families, specifically addressing adjustment issues and enhancing overall well-being.
• Collaborate with a diverse rehabilitation team, including physicians, physical therapists, and occupational therapists, to develop comprehensive and individualized treatment plans that incorporate patients' unique psychosocial needs.
• Assist patients in navigating community resources such as support groups, vocational rehabilitation, and housing assistance, ensuring access to essential services post-discharge.
• Accurately document interactions and update patients' records to reflect their progress, challenges, and ongoing needs in a timely manner.
• Participate actively in discharge planning, ensuring seamless transitions for patients returning home or to alternative care settings, advocating for their needs throughout the process.

Patient Population / Client Focus

The patients you will serve come from diverse backgrounds and may include individuals recovering from strokes, orthopedic surgeries, spinal cord injuries, neurological disorders, and other medical conditions. Your role as an LCSW will be pivotal in addressing their emotional, social, and practical challenges, working toward their reintegration into the community.

Qualifications

• A Master’s Degree in Social Work from an accredited institution is required.
• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of New York.
• Strong communication and interpersonal skills, fostering trust and rapport with patients and families.
• A thorough understanding of adjustment issues related to rehabilitation, grief, and coping strategies, enhancing your effectiveness in patient interactions.

Preferred Experience

• At least three years of experience working in a rehabilitation or medical setting, providing insight into the unique challenges faced in recovery.
