Key Responsibilities

• Conduct thorough psychosocial assessments to identify and address the emotional, social, and financial needs of patients, developing individualized care plans based on findings.
• Provide both individual and group counseling sessions, focusing on issues such as grief, adjustment disorders, and coping strategies.
• Work collaboratively with medical staff to integrate findings from psychosocial assessments into the overall treatment plan, ensuring a holistic approach to patient care.
• Coordinate with local community organizations to facilitate post-discharge services, enhancing continuity of care for patients.
• Educate patients and their families about treatment options and community resources, empowering them to make informed decisions regarding their health.
• Participate actively in clinic committees focused on quality improvement initiatives, bringing your expertise in social work to enhance departmental services and patient care.
• Provide crisis intervention support, employing skills to assist patients and families in times of emotional distress.
• Maintain accurate, timely documentation in compliance with clinic policies and state regulations, contributing to the legal and operational integrity of our services.

Qualifications

• A Master’s degree in Social Work (MSW) from an accredited institution.
• Current Licensure as a Licensed Independent Clinical Social Worker (LICSW) in Washington State.

• Excellent communication and interpersonal skills, with the ability to build rapport with patients, families, and colleagues.

Preferred Experience

• A minimum of two years of experience in a healthcare or clinical setting, ideally within a community health or hospital environment.
• Experience with interdisciplinary team collaboration and knowledge of crisis intervention techniques.
• Familiarity with electronic health record (EHR) systems and documentation practices.

Compensation and Benefits
