Position Overview
We are seeking a dedicated Licensed Professional Counselor (LPC) to join our compassionate team at Congruent Health Center. This is a full-time remote position with a salary range of USD 90,000 – 100,000 per year based on experience and availability. In this role, you will be instrumental in providing mental health services and support to a diverse clientele, working collaboratively to promote overall well-being and recovery.

Work Environment
This remote position allows for flexible scheduling to accommodate both personal and professional commitments while working from the comfort of your chosen environment. Despite the remote nature of the job, you'll be part of an engaging team, participating in regular check-ins and meetings, fostering collaborative relationships with both colleagues and clients.
Key Responsibilities
- Conduct comprehensive assessments to understand and address the unique mental health needs of each patient.
- Prepare and maintain clear communication with supervisors, direct care staff, family members, and others involved in a patient’s care to inform them of progress and any necessary adjustments to treatment plans.
- Develop and provide a structured schedule for appointments, home visits, and evaluations to ensure continuous patient support and effective monitoring of progress.
- Coordinate home visits and in-home observations to gather relevant data, performing chart reviews and staff interviews to enhance patient care.
- Ensure that all program plans are implemented effectively, modifying them as necessary based on ongoing evaluations and patient feedback.
- Engage actively in Interdisciplinary Team Meetings and contribute to discussions surrounding patient care strategies and outcomes.
- Document monthly and quarterly progress notes to track developments in patient treatment and outcomes.
- Organize training sessions aimed at certifying staff in program plan application and documentation standards, ensuring compliance with organizational protocols.
- Conduct follow-up communications, including interviews and additional home visits, to provide ongoing support tailored to the individual needs of each client.
- Maintain accurate and accessible records for easy retrieval by authorized individuals, safeguarding patient confidentiality and data integrity.
- Act as a liaison between the public and mental health community, fostering relationships that facilitate community development goals.
- Assist in the creation of training materials and procedures designed to continually enhance program efficacy and service delivery.

Qualifications
- A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.
- Active and unrestricted licensure as a Professional Counselor (LPC) in Colorado is necessary.
- A minimum of 2 years of experience in a counseling role, ideally within a clinical setting, is mandatory.
- Specialized training or experience in areas such as family therapy, couples therapy, substance misuse, or trauma-informed care is strongly preferred.
- A solid understanding of legal and ethical standards in mental health care is essential.
- Exceptional interpersonal and communication skills are critical for establishing rapport with clients from diverse backgrounds.
- Demonstrated ability to collaborate effectively with a multidisciplinary healthcare team is required.
- Familiarity with electronic medical record (EMR) software and related technological tools is advantageous.
Preferred Experience
- Experience working with various populations, especially those dealing with complex trauma or substance use disorders.
- A proven track record of success in implementing evidence-based therapeutic interventions.
- Previous experience conducting training for staff or peers in counseling methodologies is a plus.
