We’re looking for a Clinician I – School based to join us at our Family Center facility at 1591 Taos Rd Pueblo, Co 81006 Servicing various schools in District 60
What You’d Be Doing
As a School-Based Clinician with The Family Center, you are a vital member of a team that works within the students’ familiar school environments to engage youth in their journey to mental wellness. Your collaborative work with school staff and other community resources further enhances the overall wellbeing of the students, their families, and our community both now and in the future. Under the supervision of the Supervisor and Program Director, you will:
RESPONSIBILITIES:
• Facilitate quality individual, family and group therapies among clients ranging in ages from 4 to 18 in various school settings;
• Work within schools to help provide observations and/or attend school meetings of clients (i.e., 504's, RTI's, IEP's);
• Conduct intake assessments, employing professional interpretation and clinical analysis to develop effective treatment plans;
• Provide individual, family, and group therapy at the Family Center as needed;
• Conduct case management in collaboration with resources in the community;
• Provide crisis intervention/crisis response at schools as needed to support students/school staff;
• Provide consultation and education services to teachers, counselors and/or administrators as needed/requested;
• Conduct case management in collaboration with resources in the community;
• Conduct individual, family and group, sessions at two to three embedded schools per week;
• Maintain accurate and up-to-date records with 100 percent compliance with Health Solutions’ standards;
• Maintain 100 percent productivity for billable services;
• Maintain 100 percent of expectations for timely documentation for all services/outreaches conducted;
• Maintain proficiency with the electronic health record; and
• Perform other job-related duties as assigned by the Program Director or Supervisor.
