Work Environment

You will work onsite in our outpatient office setting, where a collaborative and supportive atmosphere will empower you to thrive as a healthcare provider. Your work schedule will typically range from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending to 8:00 p.m. Participation in community outreach events and health fairs on weekends may also be part of your responsibilities. This position offers an average workweek of approximately 40 hours, with a focus on maintaining a healthy work-life balance.

Key Responsibilities

• Conduct in-depth health assessments and screenings for patients, identifying both immediate and long-term healthcare needs.
• Diagnose and manage acute and chronic conditions, implementing preventive care strategies and health promotion initiatives tailored to our patient population.
• Collaborate with community organizations to develop and implement impactful health education programs aimed at raising awareness and enhancing health literacy.
• Provide culturally sensitive care and act as an advocate for patients' rights, ensuring they have access to the necessary healthcare services.
• Oversee referrals to specialists and coordinate follow-up care, reinforcing continuity of care for patients throughout their healthcare journey.
• Maintain detailed, accurate patient records in compliance with regulatory standards and ensure effective communication with healthcare team members.
• Engage actively in community outreach and health fairs, conducting health screenings and providing education about health resources available in the area.
• Stay abreast of community health trends, implement evidence-based practices into your patient care, and familiarize yourself with relevant healthcare policies.

Qualifications

• Current certification as a Nurse Practitioner.

Compensation and Benefits
