Work Environment

This position is stationed on-site in a local school, facilitating direct access to health services for students, educators, and their families. The typical work schedule runs from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m., ensuring flexibility in meeting patient needs. Participation in community outreach events and health fairs, often held on weekends, is also a vital aspect of this role, promoting health and wellness beyond the school walls.

Key Responsibilities

• Conduct comprehensive health assessments and screenings tailored to the specific needs of students and their families.
• Diagnose and manage both acute and chronic medical conditions, offering preventive care strategies and health promotion programs.
• Collaborate with local schools and community organizations to develop tailored health education programs aimed at improving health literacy.
• Deliver culturally sensitive care that acknowledges and respects the diverse backgrounds of our patients while advocating for their healthcare rights.
• Coordinate referrals to specialty services and ensure seamless follow-up care, supporting patients through their healthcare journey.
• Maintain meticulous medical records in compliance with both internal and regulatory standards, utilizing electronic health record (EHR) systems effectively.
• Actively participate in community outreach efforts by offering health screenings and wellness education during events such as health fairs.
• Regularly review current community health trends, relevant policies, and best practices to provide the highest standard of care.

Patient Population / Client Focus

Qualifications

• Must possess a current certification as a Nurse Practitioner.
