Work Environment

This position is onsite at a school-based health clinic, where you will interact with a diverse patient population in a dynamic and collaborative setting. Key Responsibilities

• Conduct thorough patient assessments, including taking detailed medical histories and performing physical examinations to evaluate health risks, concerns, and needs.
• Formulate and implement individualized care plans based on evidence-based practices, ensuring that patients receive appropriate screenings and preventative measures.
• Perform daily patient rounds, monitoring progress and adjusting care plans as necessary in collaboration with the healthcare team.
• Order and interpret diagnostic tests and laboratory results, ensuring timely communication of findings to patients and family members.
• Manage patient care continuity by providing discharge summaries, discussing follow-up appointments, and offering education on post-care instructions.
• Engage in routine meetings with clinical staff to review patient care protocols and enhance service delivery.
• Document all patient interactions accurately and timely in electronic health records, maintaining compliance with legal and ethical standards.

Patient Population / Client Focus

The Nurse Practitioner will primarily care for adolescent and adult female patients within a school-based health framework. This includes addressing a range of health issues from reproductive health education, contraceptive counseling, and managing chronic conditions. Understanding the social and educational barriers faced by this demographic will allow for a tailored approach to their health care.

Qualifications

• A Master's Degree in Nursing with a focus in Nurse Practitioner Program, accompanied by National Board Certification in a relevant specialty area.
• Active Nurse Practitioner license in New Jersey, as well as a valid Registered Professional Nurse license.
• A collaborative practice agreement with established protocols certified with the New Jersey Department of Education, or a similar attestation per the Nurse Practitioner Modernization Act.
• Current Basic Life Support (BLS) certification.

Preferred Experience

A minimum of 1 year of work experience in a nurse practitioner role, especially within women's health or pediatrics, is preferred. Familiarity with school health systems and working with adolescent populations will be highly advantageous.

Compensation and Benefits
