Work Environment

This position is based onsite at our Boston facility. You will work within a supportive team environment, collaborating directly with other healthcare professionals, community organizations, and stakeholders. The work schedule typically ranges from 9:00 a.m. to 6:00 p.m., with the possibility of occasional evening shifts until 8:00 p.m. Participation in community outreach initiatives and health fairs during weekends is also expected, further enhancing your connection to the community.

Key Responsibilities

• Conduct thorough health assessments and screenings for patients, creating personalized care plans that address both acute and chronic medical conditions.
• Diagnose and manage a variety of health issues while promoting preventive care and healthy lifestyle choices.
• Work collaboratively with local organizations to design and implement impactful health education programs tailored to the needs of the community.
• Provide culturally sensitive care, ensuring all patients receive the rights and access to the services they require.
• Coordinate referrals and follow-up care, facilitating continuity of care that enhances patient health outcomes.
• Maintain comprehensive and accurate patient records in line with regulatory standards, utilizing our electronic health record (EHR) system for efficient documentation.
• Stay informed on community health trends, public health policies, and emerging evidence-based practices that may influence care delivery.

Qualifications

• A current certification as a Nurse Practitioner.

Compensation and Benefits
