Work Environment

This position operates onsite, emphasizing a hands-on approach to patient care. Working hours typically range from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. You may also engage in community outreach events or health fairs over the weekend, averaging approximately 40 hours per week. Key Responsibilities

• Perform thorough health assessments and screenings, actively engaging with patients and families to compile detailed medical histories and health status evaluations.
• Diagnose and manage a wide range of acute and chronic medical conditions, focusing on preventive care and health promotion strategies.
• Collaborate with community organizations and local stakeholders to create and implement targeted health education programs, addressing specific community needs.
• Advocate for patients by providing culturally sensitive care, ensuring they understand their rights and facilitating access to necessary healthcare services.
• Coordinate referrals and follow-up care effectively, ensuring all patients have the continuity of care they require for optimal health outcomes.
• Maintain accurate and up-to-date medical records following regulatory standards and best practices.
• Participate actively in community outreach events and health fairs, providing health screenings and educational resources.
• Remain informed on community health trends, evidence-based practices, and health policy changes affecting patient care.

Patient Population / Client Focus

You will work closely with a diverse range of individuals and families, particularly focusing on underserved populations within Bridgeport. Your role will require you to develop an understanding of the unique health challenges faced by these communities and tailor your approach to address their specific needs, ultimately contributing to improved health outcomes and wellbeing.

Qualifications

• Valid and current certification as a Nurse Practitioner.
• An active state nursing license specific to Connecticut.
• A Master’s degree in nursing from an accredited program.
• At least 3-5 years of clinical experience, ideally within community health or public health settings.
• Strong interpersonal and communication skills, capable of fostering relationships with diverse populations.
• A commitment to social justice, health equity, and the advocacy of vulnerable communities.

Preferred Experience

• Experience in a primary care setting with a focus on family health.

Compensation and Benefits

We are proud to offer a competitive salary range of USD 180,000 to 200,000 annually, commensurate with experience and qualifications.
