Work Environment

This role is designed for an onsite practitioner who will spend the majority of their time in the field, conducting house calls and engaging directly with patients in various community settings. Your work hours will typically be from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending to 8:00 p.m. Additionally, some weekends may be designated for participation in community outreach initiatives, such as health fairs. Expect an average workweek of approximately 40 hours.

Key Responsibilities

• Perform comprehensive health assessments and screenings for patients in their homes, gathering vital health history and setting personalized health goals.
• Diagnose and manage a wide array of acute and chronic medical conditions while maintaining a focus on preventive care and health promotion strategies.
• Collaborate effectively with local community organizations to design and implement health education programs that target public health issues.
• Advocate for patients’ rights by providing culturally sensitive care and ensuring equitable access to necessary healthcare services.
• Coordinate referrals and follow-up care, creating a seamless healthcare experience for patients and ensuring continuity of their treatment plans.
• Maintain accurate and thorough medical records, adhering to all regulatory standards and best practices in documentation.
• Participate in community outreach events, offering health screenings and education to promote preventive health measures.
• Stay informed on current health trends, evidence-based practices, and relevant healthcare policies to enhance service delivery and care quality.

Patient Population / Client Focus

You will primarily serve families and individuals from diverse backgrounds in the Syracuse area. The patient population may include underserved communities with various socio-economic challenges. Your role will not only focus on their immediate health needs but will also promote long-term wellness through education and advocacy, directly impacting their health behaviors and outcomes.

Qualifications

• Must possess current certification as a Nurse Practitioner, valid in the state of New York.
• Active state nursing license, demonstrating compliance with all professional requirements.
• A Master’s degree in Nursing from an accredited program is required.
• A minimum of 3-5 years of clinical experience is necessary, preferably in community health or public health settings.
• Exceptional interpersonal and communication skills, with a strong ability to connect with diverse populations.

Preferred Experience

Compensation and Benefits

• The competitive salary range for this role is between $180,000 and $200,000 annually, based on experience and qualifications.
