Key Responsibilities

• Conduct comprehensive health assessments, including physical exams and developmental screenings, for students to identify health issues and needs.
• Facilitate referrals to other healthcare professionals and medical facilities as necessary, ensuring a smooth process for students requiring specialized care.
• Communicate effectively with collaborating physicians and specialists regarding patient care, ensuring cohesive treatment plans that address the educational and health needs of each student.
• Engage with clinical staff to coordinate patient care, including educating them about specific student health needs and supporting interventions.
• Document patient histories and treatment plans meticulously in electronic health records (EHR) systems to contribute to continuity of care.
• Participate in school health programs and initiatives, providing health education and resources to students and families to foster healthy habits.
• Stay informed on health trends within the school environment and collaborate with school administrators on health-related policies and programs.
• Perform other job-related duties as assigned to meet the evolving needs of the organization and the patient population.

Patient Population / Client Focus

This role primarily serves the student population within the school setting, addressing both acute and chronic healthcare issues, mental health support, and preventive care initiatives. The Nurse Practitioner plays a critical role in advocating for the health and well-being of students, including those with special needs or those facing social determinants of health that may impact their educational experience.

Qualifications

• Currently holds a Master of Science in Nursing, Doctor of Nursing Practice, or appropriate NP Certificate Program.
• Possesses national certification from either the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• Active, unrestricted Nurse Practitioner license in the state of New York.
• Authorization to work in the United States.
• Demonstrated ability to work effectively in a flexible, fast-paced school environment while maintaining a commitment to patient care.
• Excellent written and verbal communication skills, with a strong focus on building rapport with young patients and their families.

Preferred Experience

• Previous experience as a Nurse Practitioner in a school setting or with pediatric populations is highly desirable.
• Familiarity with electronic health records (EHR) systems and documentation standards specific to educational environments.
• Experience in conducting health education programs and initiatives to promote school-wide wellness.
