Advanced Cardiology PC is seeking a dedicated and passionate Nurse Practitioner to join our team in the Bronx, New York. This full-time role is focused on providing exceptional primary care to adults on Medicare through an innovative concierge medicine approach. The successful candidate will be responsible for independent patient management, collaborating closely with physicians, and ensuring a holistic, patient-centric care experience.

Work Environment

Working onsite in a dynamic and supportive environment, our Nurse Practitioners collaborate with a dedicated team that includes physicians, Physician Assistants, medical assistants, and support staff such as medical scribes and registered nurses. In this collaborative model, our team focuses on delivering primary care, care coordination, and population health initiatives while partnering with a network of elite specialists for any specialty or acute care needs.

Key Responsibilities

• Provide exceptional and compassionate primary care services to patients, ensuring a thorough understanding of their health needs.
• Conduct routine office visits to address both routine and urgent health issues, offering timely and effective solutions.
• Administer annual wellness visits and health risk assessments, requiring a comprehensive view of patients' overall health and precise documentation to track their progress.
• Coordinate patient care with a diverse array of healthcare providers, specialists, testing facilities, and local agencies to enhance patient outcomes.
• Lead population health initiatives in conjunction with the care team, ensuring alignment with best practices, such as facilitating regular screenings as per evidence-based guidelines.
• Support the care team with phone triage and outreach activities, ensuring that all patients receive timely communication regarding their health management.
• Educate patients about their health conditions, care plans, and treatment options to promote informed decision-making and active participation in their health journey.
• Participate in community outreach and promotional activities to enhance our visibility and support our organizational mission.
• Conduct home visits when necessary to provide care to patients who may have mobility challenges or other barriers to accessing healthcare.
• Report directly to the Medical Director and maintain close collaboration with operational leadership.

Patient Population / Client Focus

This role primarily focuses on older adults, particularly those on Medicare, who require dedicated attention to maintain their health and improve their quality of life. Our care model is designed around the features of concierge medicine, ensuring that each patient receives highly personalized services aimed at enhancing their overall wellness.

Qualifications

• A Master's Degree in Nursing, a Doctor of Nursing Practice (DNP), or completion of a Nurse Practitioner certificate program.
• Current national certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through recognized organizations such as the AANP or ANCC.
• An active, non-probationary Nurse Practitioner license in the state of New York.
• Possession of an active DEA license.
• A solid background in primary care settings, particularly in internal medicine, geriatrics, or family medicine.
• Familiarity with HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and health risk assessments.
• Strong collaborative skills and a deep passion for teamwork and patient care.

Preferred Experience

• Prior experience in concierge medicine or similar healthcare delivery models is highly desirable.
• Demonstrated ability to manage diverse and complex health conditions among older adult populations.
• Skills in using electronic health record systems for efficient patient documentation and communication.

Compensation and Benefits
