Primary Care Nurse Practitioner - Full-Time Position

Advanced Cardiology PC is seeking dedicated and passionate Nurse Practitioners to join our team in Seneca Falls, New York. This full-time, onsite role is essential in delivering high-quality, value-based primary care to our adult patients, particularly those on Medicare. With a focus on patient-centered care, our Nurse Practitioners will work independently and collaboratively with physicians to ensure comprehensive, continuous care that fosters strong patient relationships and improves health outcomes.

Work Environment

This position is based in a school setting, providing a unique opportunity to work with patients in their community. You'll be part of a multidisciplinary care team that includes a Physician, other Nurse Practitioners, Physician Assistants, Medical Assistants, and Registered Nurses, along with dedicated medical scribes. Together, you will contribute to a supportive and collaborative environment that maximizes patient health and well-being. The typical schedule involves working Monday through Friday from 9:00 AM to 5:00 PM, providing a balanced work-life dynamic.

Key Responsibilities

• Deliver exceptional primary care services to adult patients, ensuring their health needs are met effectively.
• Conduct thorough office visits to address both routine and acute medical issues, applying clinical knowledge to assess patient conditions.
• Perform annual wellness visits and health risk assessments that require meticulous documentation and a holistic view of patient health.
• Facilitate care coordination with external specialists, testing facilities, and community agencies, ensuring that patients receive the right care at the right time.
• Lead population health initiatives, such as organizing screenings for eligible patients, and advocating for preventive health measures.
• Assist the care team with phone triage, follow-ups, and outreach to enhance patient engagement and satisfaction.
• Educate patients about their health conditions, treatment options, and care plans to empower them in managing their health.
• Participate in community outreach and promotional activities to extend our services and enhance patient awareness.
• Conduct home visits as necessary to provide care for patients who may have difficulty accessing our facilities.
• Report directly to the Medical Director and collaborate closely with operational leadership to streamline care processes.

Patient Population / Client Focus

Your primary focus will be on older adults enrolled in Medicare, who often face unique health challenges. The goal is to help these patients live full, vibrant lives by providing personalized care that addresses their specific needs, preferences, and health goals. Your role will be pivotal in reducing hospital admissions and improving the quality of life for this patient population.

Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of a Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner from the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• An active, non-probationary Nurse Practitioner license in the state of New York.
• An active Drug Enforcement Administration (DEA) license to prescribe controlled substances.
• Proven experience in primary care settings, particularly in internal medicine, geriatrics, or family medicine.
• Familiarity with Hierarchical Condition Categories (HCC) documentation, ICD-10 coding, and conducting comprehensive Health Risk Assessments (HRAs).

Preferred Experience

• Experience in implementing patient education initiatives or wellness programs.
• Knowledge of community resources and health care services available for older adults.
• Demonstrated ability to work effectively within a multi-disciplinary team.

Compensation and Benefits

The role offers a competitive salary ranging between $120,000 and $160,000 annually, commensurate with experience and qualifications.
