Advanced Cardiology PC is seeking dedicated and passionate Nurse Practitioners to join our team in Seneca Falls, New York. This full-time, onsite role is essential in delivering high-quality, value-based primary care to our adult patients, particularly those on Medicare. This position is based in a school setting, providing a unique opportunity to work with patients in their community. You'll be part of a multidisciplinary care team that includes a Physician, other Nurse Practitioners, Physician Assistants, Medical Assistants, and Registered Nurses, along with dedicated medical scribes. Together, you will contribute to a supportive and collaborative environment that maximizes patient health and well-being. The typical schedule involves working Monday through Friday from 9:00 AM to 5:00 PM, providing a balanced work-life dynamic.
Your primary focus will be on older adults enrolled in Medicare, who often face unique health challenges. The goal is to help these patients live full, vibrant lives by providing personalized care that addresses their specific needs, preferences, and health goals. Your role will be pivotal in reducing hospital admissions and improving the quality of life for this patient population.
The role offers a competitive salary ranging between $120,000 and $160,000 annually, commensurate with experience and qualifications.
