We are seeking a dedicated and compassionate Family Medicine Nurse Practitioner for a full-time role at Brightwood Orthopedic and Family Medicine Care in Brookhaven, New York. Our Nurse Practitioners work both independently and collaboratively with physicians, enabling them to design tailored treatment plans that enhance patient outcomes.

Work Environment

The successful candidate will be part of an onsite outpatient practice in Brookhaven, NY, where you will work from Monday to Friday, 8:00 AM to 5:00 PM. You will engage with a dynamic and collaborative care team consisting of physicians, physician assistants, nurse practitioners, medical assistants, a dedicated medical scribe, and support staff, all working together to focus on primary care, care coordination, and population health strategies.

Key Responsibilities

• Deliver high-quality primary care that addresses both routine and acute medical issues.
• Conduct comprehensive annual wellness visits and health risk assessments, ensuring accurate and holistic documentation of patient health.
• Coordinate care across multiple providers, specialists, and agencies to facilitate efficient patient services.
• Lead initiatives focused on population health, including ensuring eligible patients receive timely screenings such as evidence-based breast cancer assessments.
• Engage with patients actively through phone triage, outreach efforts, and education regarding their health status and treatment options.
• Participate in promotional activities to support the organization’s growth and community engagement.
• Perform home visits as necessary to deliver care to patients with mobility challenges or complex needs.
• Report directly to the Medical Director while closely collaborating with operational leadership to maintain high standards of care.

Patient Population / Client Focus

Your primary focus will be on providing individualized and empathetic care to older adults on Medicare, with a commitment to understanding their unique health challenges. By building trusting relationships with patients, you will empower them to make informed decisions about their health and care plans, thereby enhancing their quality of life.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or graduate from an NP Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• Active, non-probationary state Nurse Practitioner license and DEA license.
• Experience in primary care settings such as internal medicine, geriatrics, or family medicine.
• Familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs).
• A collaborative spirit and a passion for teamwork in a patient-centered care environment.

Preferred Experience

• Bilingual proficiency in relevant languages is highly desirable.
• A demonstrated interest in population health initiatives and innovative care models.

Compensation and Benefits

This role offers a competitive salary range of USD 115,000 to 150,000 annually, which may be adjusted based on experience and expertise. Additionally, you will have access to a performance-based quarterly bonus based on quality metrics.
