We are seeking a dedicated and compassionate Family Medicine Nurse Practitioner for a full-time role at Brightwood Orthopedic and Family Medicine Care in Brookhaven, New York. This position offers an opportunity to provide exceptional primary care to adults, focusing on holistic patient health and wellness. Our Nurse Practitioners work both independently and collaboratively with physicians, enabling them to design tailored treatment plans that enhance patient outcomes.

The successful candidate will be part of an onsite outpatient practice in Brookhaven, NY, where you will work from Monday to Friday, 8:00 AM to 5:00 PM. You will engage with a dynamic and collaborative care team consisting of physicians, physician assistants, nurse practitioners, medical assistants, a dedicated medical scribe, and support staff, all working together to focus on primary care, care coordination, and population health strategies.
Your primary focus will be on providing individualized and empathetic care to older adults on Medicare, with a commitment to understanding their unique health challenges. By building trusting relationships with patients, you will empower them to make informed decisions about their health and care plans, thereby enhancing their quality of life.
This role offers a competitive salary range of USD 115,000 to 150,000 annually, which may be adjusted based on experience and expertise. Additionally, you will have access to a performance-based quarterly bonus based on quality metrics.
