Work Environment

This position is primarily onsite, featuring a collaborative healthcare setting. You will work alongside a supportive team of healthcare professionals, including physicians, nurse practitioners, physician assistants, medical assistants, and dedicated medical scribes. Together, this multidisciplinary team fosters a patient-centered approach, enabling efficient care coordination and comprehensive patient management. Our partnerships with high-caliber specialists and hospitals amplify our ability to address both routine and acute care needs effectively.

Key Responsibilities

• Deliver exceptional primary care to a diverse patient population focused on long-term wellness.
• Conduct in-depth office visits for both routine and acute health issues, utilizing a thorough approach to patient assessment.
• Administer annual wellness visits and comprehensive health risk assessments that incorporate holistic views of patient health.
• Facilitate care coordination efforts among healthcare providers, specialists, diagnostic testing facilities, and ancillary services.
• Lead population health initiatives by ensuring all eligible patients receive evidence-based screenings and interventions.
• Engage in phone triage and proactive outreach to enhance patient care and follow-up.
• Educate patients regarding their health conditions, proposed care plans, and necessary treatments to foster understanding and compliance.
• Participate in community outreach and promotional activities that elevate our brand and mission.
• Conduct home visits to assess patient health needs and provide care as necessary.
• Collaborate closely with the Medical Director and operational leadership to align clinical practices with organizational goals.

Patient Population / Client Focus

Your primary focus will be on older adults on Medicare, a population that requires nuanced, patient-driven care. You will help them navigate their healthcare options, understand their treatment plans, and engage actively with their health goals. Your efforts in establishing trust and communication will empower these individuals to take charge of their health, thereby significantly improving their quality of life.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• An active, non-probationary state Nurse Practitioner license.
• Possession of an active DEA license.
• A strong background in primary care, with significant understanding of internal medicine, geriatrics, or family medicine.
• Working knowledge of HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs).
• A collaborative spirit and a desire to work cross-functionally within a healthcare team.
• Bilingual skills in relevant areas are preferred, enhancing communication with our diverse patient population.

Preferred Experience

• Prior experience in a primary care setting, particularly with older adult populations.
• Experience in managing chronic diseases and implementing preventative care strategies.
• Familiarity with electronic health record (EHR) systems and telehealth technologies.
• Proven track record of promoting health literacy and patient engagement initiatives.

Compensation and Benefits

This position offers a competitive salary range of USD 120,000 to 160,000 per year, commensurate with experience and expertise. In addition to a robust base salary, we provide a comprehensive benefits package that includes:
