Healing Hands Healthcare Center is seeking a dedicated full-time Nurse Practitioner to join our team in Chicago, Illinois. Work Environment

This position is based onsite at our outpatient office in Chicago and offers a full-time schedule, typically from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Flexibility may be required for participation in community outreach events or health fairs, often scheduled on weekends. You will be part of a dynamic team environment, working alongside healthcare professionals who are equally passionate about community health and patient advocacy.

Key Responsibilities

• Perform comprehensive health assessments and screenings for patients, utilizing advanced diagnostic skills to evaluate both acute and chronic health issues.
• Develop and implement individualized care plans, with a focus on preventive care practices and health promotion tailored to the unique needs of each patient.
• Collaborate with community organizations and local stakeholders to create and execute impactful health education programs, enhancing community knowledge and resources related to health and wellness.
• Advocate for patients' rights and ensure culturally competent care, actively addressing barriers to healthcare access within diverse populations.
• Oversee coordination of referrals to specialists and follow-up care, ensuring continuity and quality of care throughout the patient journey.
• Maintain detailed and accurate medical records in compliance with healthcare regulations, ensuring high standards of documentation and patient privacy.
• Engage in community outreach initiatives and health fairs, offering screenings and health education to promote preventive care and early intervention.
• Stay informed of emerging community health trends, robust evidence-based practices, and relevant local/state health policies to enhance care delivery.

Patient Population / Client Focus

The Nurse Practitioner will primarily serve a diverse patient population in Chicago, including individuals and families from various cultural and socioeconomic backgrounds. Qualifications

• A Master's degree in Nursing from an accredited institution is required.
• Current Nurse Practitioner certification and active state nursing license are mandatory.

• Proficient interpersonal and communication skills are essential, with an ability to build rapport and trust with diverse patients and their families.

Preferred Experience

Compensation and Benefits

• The salary for this role ranges from $120,000 to $140,000 annually, commensurate with experience and qualifications.
