Work Environment

The work setting is designed for optimal patient care and collaborative teamwork. Our office operates during the day from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending to 8:00 p.m. You will have a typical workload of approximately 40 hours per week, with potential involvement in community outreach activities and health fairs during weekends to further extend our outreach capabilities.

Key Responsibilities

• Conduct detailed health assessments and screenings for patients of all ages, ensuring that care is tailored to meet their individual needs.
• Expertly diagnose and manage both acute and chronic medical conditions, with a strong focus on preventive care and health promotion activities.
• Collaborate with local organizations and stakeholders to design and implement impactful health education programs, workshops, and seminars aimed at improving community health.
• Advocate for patient rights, providing culturally sensitive care that meets diverse cultural and socio-economic contexts.
• Oversee referral processes and follow-up care to guarantee seamless continuity of care for your patients.
• Maintain precise and thorough medical records in accordance with healthcare regulations and best practices.
• Engage actively in community outreach initiatives such as health fairs, where you will provide essential health screenings and educational resources.

Patient Population / Client Focus

Qualifications

• Current and valid certification as a Nurse Practitioner.
• An active nursing license in the state of New York.

• Strong interpersonal and communication skills, enabling effective collaboration with diverse populations and team members.

Preferred Experience
