Work Environment

The successful candidate will operate onsite and will engage directly with patients in their homes or in convenient community settings. With a structured schedule from 9:00 a.m. to 6:00 p.m. during weekdays and occasional evening clinics, as well as participation in community outreach events, you will enjoy a balanced work environment that fosters both professional growth and meaningful community engagement. Key Responsibilities

• Conduct thorough health assessments and screenings, utilizing tools that support accurate diagnosis and effective treatment plans tailored to each patient's individual needs.
• Diagnose and manage a wide range of acute and chronic medical conditions, providing preventive care and health promotion strategies informed by current clinical guidelines.
• Collaborate with local community organizations and stakeholders to design and implement impactful health education programs that promote wellness and healthy lifestyle choices.
• Deliver culturally sensitive care that respects the diverse backgrounds of patients, ensuring that health equity is at the forefront of your practice.
• Coordinate referrals and follow-up care, working with specialists and community resources to ensure that all patients receive comprehensive and continuous care.

Qualifications

Compensation and Benefits

We offer a competitive annual salary ranging from $180,000 to $200,000, commensurate with experience and qualifications.
