Key Responsibilities

• Conduct comprehensive assessments and diagnose patients with both acute and chronic medical conditions, ensuring accurate treatment and care plans.
• Develop, implement, and modify individualized treatment plans tailored to the specific needs of patients, which may include ordering diagnostic tests and interpreting their results.
• Prescribe appropriate medications and therapies while considering patient history and current health needs.
• Provide health education and counseling to patients and their families, focusing on disease prevention, lifestyle modifications, and health promotion.
• Collaborate seamlessly with physicians, nursing staff, and other specialists to ensure continuity of patient care and optimize health outcomes.
• Maintain meticulous medical records in compliance with HIPAA regulations and assist in quality improvement initiatives to enhance patient safety and care standards.
• Engage in ongoing education and training, keeping abreast of the latest healthcare trends and regulatory requirements affecting nursing practice.

Patient Population / Client Focus

You will engage with a diverse range of patients, including those seeking rehabilitation following surgery, managing chronic pain, or participating in wellness programs. Your approach will focus on the whole patient, prioritizing not only immediate care needs but also long-term health and recovery goals.

Qualifications

• Current certification as a Nurse Practitioner from an accredited institution.
• Active nursing license to practice in the state of Oregon.
• A minimum of a bachelor’s degree in nursing; a master’s degree is preferred.
• A solid foundation with at least 3 years of hands-on clinical experience in a healthcare setting, particularly in areas related to rehabilitation, wellness, or chronic disease management.
• Demonstrated excellent communication skills, both verbal and written, as well as strong interpersonal abilities to foster trusting relationships with patients.
• Analytical mindset with the capability to assess health situations, identify problems, and devise effective solutions.

Preferred Experience

• Experience with electronic health record (EHR) systems and telehealth technologies.
• Previous work in a collaborative multi-disciplinary team environment.
• Continuing education in specialized areas such as pain management, physical rehabilitation, or lifestyle coaching.

Compensation and Benefits

This position offers a competitive salary range of USD 150,000 to 180,000 annually, commensurate with experience and qualifications. In addition to a robust salary, we provide a comprehensive benefits package including:
