Heritage Valley Hospital is seeking a compassionate and dedicated Nurse Practitioner (NP) to join our exceptional home care team in Huntington, New York. This full-time role offers a unique opportunity to make a significant impact on patient care by delivering direct health services in a home setting. The NP will work alongside a collaborative team of healthcare professionals from Monday to Friday, 9 AM to 5 PM, ensuring comprehensive health assessments, diagnoses, and patient management solutions for those requiring home-based care.

This position is an onsite role within our home care facility, providing services directly to patients in their residences. The Nurse Practitioner will primarily serve patients in their homes, with a focus on individuals who may have mobility challenges, acute health episodes, or chronic conditions requiring ongoing management. You will encounter a diverse patient demographic, including elderly individuals, those with complex health needs, and patients recovering from surgeries who benefit from personalized follow-up care in a comfortable environment.
The compensation package for this role ranges from $180,000 to $200,000 annually, commensurate with experience and skills, coupled with potential bonuses linked to productivity and quality performance. Benefits include:
