Work Environment

This role is based onsite, primarily involving patient interactions in the home setting. The work schedule follows a traditional Monday to Friday routine from 9 AM to 5 PM, allowing for a balance between professional responsibilities and personal life. As a Nurse Practitioner at Nuvina Health, you will engage directly with patients and their families, creating a supportive environment to foster healing and recovery.

Key Responsibilities

• Conduct thorough assessments of patients in their homes, including taking medical history, performing physical examinations, and interpreting diagnostic tests.
• Develop and implement comprehensive, individualized care plans in conjunction with supervising physicians and healthcare team members, ensuring all patient needs are met.
• Manage ongoing patient care, including diagnosing conditions, ordering necessary tests, interpreting results, and initiating appropriate treatment plans.
• Assist in performing procedures pertinent to home care, such as medication administration and wound care management.
• Conduct routine follow-up visits, monitor patient progress, and adjust care plans as necessary to ensure optimal patient outcomes.
• Provide education to patients and their families regarding treatment options, medication management, and effective recovery strategies tailored to each individual.
• Collaborate with nurses, case managers, and specialists to coordinate care and ensure patients receive a holistic approach to their healthcare needs.
• Maintain accurate and timely documentation of patient interactions and care plans in our electronic health record (EHR) system.

The focus is on home care, which includes a diverse patient population, such as those managing chronic illnesses, recovering from surgeries, or needing specialized care in a comfortable, familiar environment. Qualifications

• A Bachelor’s degree in Nursing and a Master's degree from an accredited Nurse Practitioner program.
• Current or eligibility for Nurse Practitioner licensure in Illinois.
• Board certification as a Nurse Practitioner in a relevant specialty such as Family, Adult-Gerontology, or Acute Care.
• Strong decision-making skills and the ability to practice independently while collaborating with supervising physicians.
• Excellent interpersonal and communication skills for effective patient and team interactions.
• Detail-oriented with a commitment to providing compassionate, patient-centered care.

Preferred Experience

• At least 2 years of experience as a Nurse Practitioner in a home care or acute care setting.
• Experience working collaboratively within a multidisciplinary healthcare team.
• Comfort and familiarity with electronic health records (EHR) and other relevant medical software.
• Previous exposure to patient education and engagement strategies.

Compensation and Benefits
