Work Environment

Located in Decatur, Illinois, our practice adopts a concierge medicine approach that distinguishes us from traditional healthcare settings. As an NP at Nuvina Health, you'll operate primarily in an onsite capacity, facilitating house call visits while collaborating closely with attending physicians and diverse healthcare teams. Your working hours will be Monday to Friday from 9 AM to 5 PM, maintaining a balanced work-life schedule to promote your well-being alongside your patients.

Key Responsibilities

• Conduct comprehensive and empathetic assessments of patients, including detailed history-taking and thorough physical exams.
• Develop and implement individualized care plans collaboratively with fellow providers to enhance patient outcomes.
• Manage ongoing patient care by diagnosing conditions, ordering relevant tests, interpreting results, and initiating effective treatment strategies tailored to each patient.
• Assist in performing a range of minor procedures, such as central line placements, intubations, suturing, and wound care, ensuring best practices are adhered to.
• Perform daily rounds to evaluate patient progress, providing updates and adjusting care plans based on dynamic patient needs.
• Facilitate patient education by discussing treatment plans, medications, and recovery strategies to empower individuals in their health journeys.
• Collaborate with nurses, case managers, and specialists to ensure well-rounded care and management of complex health issues.
• Maintain thorough and timely documentation of patient interactions and clinical data in our electronic health record (EHR) system to support continuity of care.

Patient Population / Client Focus

Your role will primarily focus on adult and geriatric patients, many of whom may have chronic illnesses or complex medical needs requiring comprehensive care approaches. You will enhance their healthcare accessibility and comfort by providing services in the familiarity of their own environment or our welcoming facility.

Qualifications

• Master’s degree in Nursing with a valid Nurse Practitioner certification (NP-C) from an accredited institution.
• Current or eligibility for Illinois NP licensure, reflecting our commitment to compliance and high standards of medical practice.
• Board certification in a relevant NP specialty (e.g., Adult-Gerontology, Family, Acute Care).
• Strong ability to work independently while making informed clinical decisions, under the physician's guidance.
• Excellent interpersonal and communication skills to build rapport with patients and foster collaborative team environments.

Preferred Experience

• A minimum of two years of experience in a clinical setting, particularly in acute care or primary care.
• Experience with EHR systems and documentation processes to streamline patient management and communication.
• A commitment to pursuing continuous medical education and improvement to stay abreast of the latest clinical practices.

Compensation and Benefits

Nuvina Health offers a competitive salary ranging from $180,000 to $200,000 annually based on experience, along with a comprehensive benefits package.
