Work Environment

The position is onsite, predominantly within the patient's home setting. Key Responsibilities

• Conduct thorough health assessments and comprehensive physical exams, utilizing advanced clinical techniques to ensure complete understanding of patients' health statuses.
• Order, perform, and accurately interpret diagnostic tests such as lab work and imaging studies, tailoring treatment plans based on empirical evidence.
• Work collaboratively with a diverse healthcare team to develop and implement patient-centered care plans that address individual health needs and preferences.
• Prescribe necessary medications, ensuring safe and informed medication management while monitoring patient responses.
• Deliver personalized health education and counseling to patients and families, empowering them to make informed decisions about their healthcare.
• Participate actively in team meetings and case conferences, sharing insights and strategies to improve patient care coordination and outcomes.
• Maintain accurate documentation of patient encounters in compliance with regulatory guidelines and organizational standards, ensuring high-quality medical records.

Patient Population / Client Focus

This role involves caring for a diverse patient population, ranging from newborns to older adults. Qualifications

• A valid and unrestricted Nurse Practitioner license in the state of New York is required.
