Key Responsibilities

• Conduct thorough health assessments and screenings for residents, tailoring assessments to address acute and chronic medical conditions specific to the elderly population.
• Diagnose and manage medical conditions, including developing and implementing individualized care plans that promote preventive measures and health education.
• Collaborate extensively with multidisciplinary teams, community organizations, and healthcare stakeholders to develop health programs aimed at improving health outcomes and access to care.
• Advocate for residents’ rights and ensure they receive culturally sensitive and appropriate care, recognizing individual preferences and values.
• Facilitate and coordinate referrals to specialists and follow-up on care transitions to ensure a seamless care experience for residents.
• Maintain accurate and detailed medical documentation in compliance with both regulatory and organizational standards, leveraging EHR systems effectively.
• Engage in community outreach efforts, including participation in health fairs and wellness events to further educate and empower residents and their families.
• Remain updated on health trends, best practices in gerontology, and local healthcare policies affecting assisted living facilities.

Patient Population / Client Focus

The majority of the residents you will care for are seniors who may require assistance with daily activities and management of chronic health conditions, including heart disease, diabetes, and dementia. Your ability to connect with this population and adapt care strategies accordingly is vital to the success of our services. Building strong, trusting relationships with residents and their families is essential to facilitating effective care and ensuring satisfaction.

Qualifications

• Master's or Doctoral degree in Nursing from an accredited institution.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from a recognized body such as the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Possession of a current and unrestricted Nurse Practitioner (APRN) license in New York State.
• Must hold valid DEA and NPI numbers.
• Minimum of two years of clinical experience, preferably in a primary care or family medicine setting.
• Demonstrated clinical assessment and diagnostic skills specific to the geriatric population.
• Strong communication and interpersonal skills that enhance patient engagement and team collaboration.
• Ability to work independently while being an effective team player.
• Proficiency in using electronic health records (EHR) and other relevant healthcare technology.

Preferred Experience

• 3-5 years of experience in community health or public health settings, particularly with elderly populations.
• Experience in leading health education initiatives or community health programs is highly desirable.
• Strong understanding of the unique healthcare challenges faced by residents in an assisted living setting.

Compensation and Benefits

The salary for this position ranges from $150,000 to $160,000 annually, reflecting your expertise and the vital work you will perform.
