Key Responsibilities

• Conduct thorough health assessments and screenings for all residents, identifying both immediate and long-term healthcare needs.
• Diagnose, treat, and manage acute and chronic medical conditions, ensuring comprehensive preventive care and health education tailored to our elderly population.
• Collaborate with interdisciplinary teams and local health organizations to design and implement effective health programs, addressing common geriatric issues.
• Advocate for residents' rights, providing culturally competent care that respects the diverse backgrounds of the individuals we serve.
• Coordinate referrals to specialists and manage follow-up care, ensuring that residents receive continuous and effective treatment.
• Maintain accurate and confidential medical records in accordance with state regulations and organizational policies.
• Participate actively in resident care meetings, contributing insights from assessments to enhance care plans.
• Engage in community outreach initiatives to raise awareness about health resources available to the senior population.

Patient Population / Client Focus

This role specifically involves working with elderly residents in an Assisted Living environment, focusing on their unique healthcare needs. You will play a vital role in promoting health literacy among residents and their families, guiding them through the complexities of aging-related health issues.

Qualifications

• Master's or Doctoral degree in Nursing from an accredited program.
• Current, unrestricted Nurse Practitioner (APRN) license in the state of New York.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from recognized credentialing bodies.
• Possession of a valid DEA# and NPI#.
• Strong clinical assessment and diagnostic abilities, particularly in geriatric medicine.
• Excellent communication and interpersonal skills, with the ability to connect with residents and their families empathetically.

Preferred Experience

• A minimum of 2 years of experience in primary care or family medicine, preferably within an assisted living or senior healthcare environment.
• Familiarity with electronic health records (EHR) systems to streamline patient data management.
• Demonstrated ability to work effectively both independently and as part of a comprehensive care team.

Compensation and Benefits

This full-time position offers a competitive salary range of USD 150,000 – 160,000 per year. In addition to a lucrative compensation package, benefits include health insurance, retirement plans, and a generous sign-on bonus of up to $10,000.
