Key Responsibilities

• Conduct thorough assessments and accurate diagnoses of both acute and chronic medical conditions, employing tools such as patient interviews, physical examinations, and diagnostic tests.
• Formulate individualized treatment plans based on evidence-based practices while regularly reviewing and modifying these plans as necessary for optimal care delivery.
• Prescribe medications and therapeutic interventions tailored to each patient’s unique health situation, managing treatment regimens, and monitoring for effectiveness and safety.
• Educate patients and their families on disease prevention, health promotion, and management strategies, fostering a collaborative approach to health between practitioners and patients.
• Engage and collaborate with a diverse array of healthcare professionals, including physicians, nurses, social workers, and occupational therapists to coordinate comprehensive patient care.
• Maintain meticulous medical records in line with regulatory standards, ensuring accurate documentation that supports quality care and facilitates effective communication with other team members.
• Actively participate in quality improvement initiatives aimed at enhancing service delivery and outcomes for our residents.

Patient Population / Client Focus

The Health Nurse Practitioner will primarily engage with older adults residing in the assisted living facility. Many of these patients may be dealing with complex medical histories, including chronic pain, mobility challenges, and cognitive issues. As a Nurse Practitioner, you will play a vital role in comprehensively managing their healthcare needs, focusing on enhancing quality of life and promoting independence wherever possible.

Qualifications

• A current certification as a Nurse Practitioner, with active state licensure to practice in Oregon.
• A Bachelor’s degree in nursing from an accredited institution; a Master's degree in nursing is highly preferable.
• A minimum of three years of clinical experience in a relevant specialty, ideally within geriatric care or an assisted living setting.
• Exceptional communication and interpersonal skills, demonstrating the ability to build rapport with both patients and their families.
• Strong analytical and problem-solving skills, enabling effective decision-making in dynamic healthcare environments.

Preferred Experience

• Experience managing complex patient care plans in geriatric or rehabilitation settings.
• Familiarity with electronic medical records (EMR) systems, enhancing efficiency in documentation and communication.

Compensation and Benefits

We offer a competitive salary range of $150,000 to $180,000 annually, commensurate with experience and educational qualifications.
