Pulse Point Internal Medicine is seeking dedicated Full-Time Nurse Practitioners to join our innovative healthcare team located in North Chili, New York. Work Environment

This position is based onsite in our home care facility, where you will be an integral part of a collaborative care team. You will work alongside physicians, other nurse practitioners, physician assistants, medical scribes, registered nurses, and care managers. Our work setting supports a multi-disciplinary approach, allowing our healthcare providers to focus on primary care, care coordination, and population health while fostering an environment where teamwork is paramount.

Key Responsibilities

• Deliver extraordinary primary care to patients during routine and acute office visits.
• Perform comprehensive annual wellness visits and health risk assessments, ensuring accurate documentation of patient health.
• Coordinate care with other healthcare providers, specialists, testing facilities, and community agencies to ensure seamless patient experiences.
• Lead population health initiatives, such as ensuring compliance with routine screenings for eligible patients.
• Assist the care team with triaging patient calls, conducting outreach, and managing patient inquiries.
• Educate patients on their medical conditions, treatment options, and care plans to enhance their understanding and engagement.
• Participate in community outreach and promotional activities to support the organization’s growth and visibility.
• Conduct home visits when necessary to provide care in patient-friendly environments, remaining attentive to their individual health needs.
• Perform other duties as assigned, reporting directly to the Medical Director and collaborating closely with operational leadership.

Patient Population / Client Focus

Your primary focus will be on older adults enrolled in Medicare. We recognize the unique health challenges this population faces, and our innovative care model is designed to provide them with the necessary support to thrive in their communities.

Qualifications

• A Master of Science in Nursing, Doctor of Nursing Practice, or completion of an NP Certificate Program.
• Nationally certified as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• A valid, active Nurse Practitioner license in the state of New York, without probation.
• Current DEA license for prescribing medications.
• A background in primary care settings such as internal medicine, geriatrics, or family medicine.
• Familiarity with HCC documentation, ICD-10 coding, and conducting health risk assessments.

Preferred Experience

• Experience working in value-based care settings that prioritize patient outcomes.
• Demonstrated passion for team collaboration and innovation in healthcare.
• Bilingual skills are a plus, facilitating better communication with diverse patient populations.

Compensation and Benefits

The annual salary for this position ranges from USD 180,000 to 200,000, with opportunities for a wider range based on experience and expertise. In addition to competitive pay, we offer an appealing benefits package that includes:
