Work Environment

This role is onsite, allowing for the delivery of healthcare in the comfort of patients' homes or at our clinic. Key Responsibilities

• Prescribe medications tailored to each patient's circumstances while considering efficacy, safety, and cost.
• Develop personalized treatment plans that incorporate behavior modification strategies for health improvement.
• Educate patients and their families about self-management of both acute and chronic conditions, ensuring instructions are clear and customized.
• Conduct thorough evaluations of patients’ medical and psychological health, determining necessary interventions for ongoing care.
• Utilize Electronic Health Records (EHR) effectively, ensuring documentation is comprehensive and compliant with relevant regulations.
• Establish medication dosages, routes, and frequencies based on individual patient characteristics and clinical guidelines.

Patient Population / Client Focus

As a Nurse Practitioner in our organization, you will work with a diverse patient population that spans all age groups. Qualifications

• National certification from the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• Current, unrestricted New York state license to practice as a Nurse Practitioner.
• Authorization to work in the United States.
• A Master's degree in Nursing, a Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner Certificate Program.

Preferred Experience

• Experience in a primary care setting or concierge medicine is preferred but not mandatory.
• Demonstrated ability to adapt to changing patient needs, environments, and schedules.

Compensation and Benefits

• A competitive yearly salary in the range of $150,000 to $160,000, determined by individual caseload.
